In this field-based role, you will complete comprehensive assessments, develop and update individualized care plans, manage pain and symptoms, and administer medications and treatments in patient homes and facilities. You will educate and support families, collaborate with an interdisciplinary hospice team, and ensure accurate, timely documentation.
