Full job description

Position Highlights
  • Full-time or part-time opportunities available
  • Round on approximately patients per day
  • Practice in Skilled Nursing Facilities (SNFs) and Assisted Living communities
  • Build lasting relationships with patients and facility teams
  • Work independently with a high level of clinical autonomy
Compensation
  • Base salary of $275,000
  • Productivity incentives with earning potential of $350,000 $400,000+ annually through RVUs
Ideal Candidate
  • Board Certified or Board Eligible in Family Medicine , Internal Medicine , or Geriatric Medicine
  • Comfortable practicing independently in post-acute care settings
  • Strong interpersonal skills with a patient-centered approach
  • Interested in developing long-term relationships within the facilities they serve
