In-Home Advanced Practice Provider

Signify Health is seeking a full-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health assessments in members’ homes. You will review medical histories, conduct physical exams, identify risk factors, and document findings in an EMR to support value-based care programs. Collaborating with interdisciplinary teams, you’ll help close care gaps, educate patients and caregivers, and coordinate follow-up care. This role blends clinical autonomy, travel to patient homes, and technology-driven care to improve outcomes for vulnerable populations.
