In this role, you will assess and treat patients across diverse settings to improve independence in daily living, cognition, and mobility. You’ll design individualized, evidence-based treatment plans, provide hands-on interventions, and educate patients and families. Collaborating with an interdisciplinary team, you’ll document in our EMR, support safe discharges, and contribute to quality initiatives in an innovative, inclusive environment with strong opportunities for growth.
