Signify Health is seeking an In-Home Physician Per Diem to perform comprehensive health evaluations for members in their homes. You will conduct detailed histories and physicals, document in a mobile EMR, reconcile medications, and identify care gaps and social determinants of health. Collaborating with interdisciplinary teams, you’ll support value-based care plans that improve outcomes for vulnerable populations.
