RN MDS Nurse

A Healthcare Facility in the Syracuse, NY area is seeking an experienced MDS Coordinator / MDS Assessor to join our clinical team.

Must Have MDS 3.0 Experience

Qualifications:

• Active New York State RN License required

• Previous experience in a clinical care coordination or assessment role preferred

• Strong organizational and time management skills

• Solid computer and documentation skills

• Excellent communication and interpersonal abilities

• Ability to work collaboratively within a multidisciplinary team

Responsibilities:

• Complete accurate assessments, MDS documentation, and care plans in accordance with regulations and facility standards

• Initiate and update individualized care plans as assigned

• Create and distribute monthly care plan schedules in a timely manner

• Maintain and update clinical assessments and documentation as required

• Monitor and audit clinical records to ensure accuracy, compliance, and timeliness

• Communicate documentation concerns and compliance issues to nursing leadership

• Maintain strict confidentiality of patient and organizational information
