Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Connecticut Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in home health delivery preferred. Candidates must be able to manage caseloads in patients' homes and document visits electronically.
• Valid driver's license and reliable transportation required for travel between patient homes. Mileage between visits will be reimbursed.
• Minimum education requirement: Bachelor's Degree. Degree in Physical Therapy required.
• Ability to work independently and coordinate care across interdisciplinary teams. Previous experience with home health documentation and care planning preferred.

• Shift Details: Days 0800-1600.
• Weekly Hours Guarantee: 32 hours per week.
• On Call/Weekend Requirement: No.
• Coverage Areas: Multiple towns in Connecticut including Mansfield, Waterford, Meriden/Wallingford, Waterbury/Naugatuck, Southbury/Newtown, Hamden, and Fairfield (candidate should expect to travel within these service areas).
• Dress Code: Scrubs or business casual.
