Licensed Practical Nurse (LPN) – Full-Time
Starting pay rate is $28.00/hour or more based on experience
$2,500 Sign-On Bonus!
Day Shift: 7:00 AM–7:00 PM
Overnight Shift: 7:00 PM–7:00 AM

As a Licensed Practical Nurse (LPN), you’ll provide hands-on nursing care, medication management, health monitoring, and education under the direction of an RN or physician.

Bonus Payout Structure
- 1 month - $300
- 3 months - $500
- 6 months - $600

What You’ll Do
As an LPN, you’ll play an important role in supporting the health, safety, and well-being of the individuals we serve. Responsibilities include:
- Provide quality nursing care and prescribed medical treatments under the direction of an RN or physician.
- Complete health assessments and communicate findings to the RN or physician to assist with developing individualized nursing care plans.
- Follow up on physician orders and ensure individuals receive necessary medical and dental care.
- Administer medications according to established guidelines and maintain accurate, timely medication records.
- Coordinate with pharmacies to support medication management and continuity of care.
- Review medication errors, identify trends, and recommend opportunities for process improvement.
- Monitor and support staff responsible for medication administration.
- Conduct quality assurance reviews of healthcare records and documentation.
- Provide emergency nursing coverage and care when needed.
- Coordinate with external healthcare providers and agencies to promote consistent, high-quality care.
- Administer, track, and maintain required testing records for staff and individuals served.
- Provide staff training on topics such as medication administration, infection control, and universal precautions.
- Educate individuals served on health-related topics, including self-medication, prescribed treatments, wellness, and prevention.
- Maintain confidentiality and uphold individual rights while following organizational policies and universal precautions.
- Assist with transportation, meal preparation, light household activities, appointments, and other program needs when necessary.
- Participate in the on-call rotation as needed.
- Perform additional nursing-related responsibilities as assigned.

Qualifications
- Associate degree in Nursing, related field, or applicable state nursing certificate
- Current LPN/LVN license in the state of practice
- At least one year of nursing experience working with the population served
- CPR/First Aid and other certifications required by applicable state licensing regulations
- Ability to travel as needed
