UPMC Hamot is hiring a Full-Time Physician Assistant or Certified Registered Nurse Practitioner to help support Cardiology!

Position Specifics: 80% outpatient clinic and 20% inpatient setting. This APP will work with the valve team specializing in heart valve disease, helping to coordinate interventions and surgical treatments for that population.

Department Details: Comprehensive Cardiac Care Center Certification. 18 physicians (consisting of both general, electrophysiology, and interventional cardiologists), 26 APPs in the cardiology practice providing care at UPMC Hamot with outreach to NW PA and NY regions. Valve team, Watchman Clinic, CHF Clinic, Cardiac Device Clinic with regional outreach.

Hours: Generally, Monday to Friday 8-hour daylight shifts, no on-call or weekends!

Location: UPMC Hamot Heart Institute (main office on 2nd floor, APP will work out of 4th floor).

Purpose:

The Valve Clinic Coordinator is responsible for coordinating and managing all administrative, scheduling, and patient navigation activities for the Valve Clinic. This role serves as a key point of contact for patients, physicians, and multidisciplinary care teams, ensuring timely evaluation, treatment planning, and follow-up for patients with structural heart and valve disease. The Coordinator oversees referral intake, appointment scheduling, diagnostic testing coordination, data tracking, and communication among providers to support high-quality patient care and an exceptional patient experience.

Key Responsibilities:

  • Coordinate patient referrals, evaluations, and follow-up appointments for the Valve Clinic.
  • Serve as the primary liaison between patients, cardiologists, cardiac surgeons, and ancillary departments.
  • Schedule and track diagnostic tests, procedures, and consultations to ensure timely care progression.
  • Maintain accurate patient records, databases, and program metrics in compliance with organizational policies.
  • Educate patients and families regarding appointments, testing requirements, and treatment pathways.
  • Monitor patient care plans and facilitate communication between interdisciplinary team members.
  • Assist with quality improvement initiatives, data collection, and reporting related to clinic performance.
  • Support regulatory, accreditation, and documentation requirements for the Valve Clinic program.

Master's degree in business, nursing, healthcare management or related field is required. CRNP or PA required. 3 progressively increasing operational experience in a corporate/professional environment required, preferably in hospital or internal medicine. 5 years preferred. Strong process management, problem solving, organizational, and communication skills (oral and written). Demonstrated initiatives and ability to work independently. Ability to manage multiple priority standard lines concurrently. The ability to perform planning for large initiatives/projects, managing all tasks required to be departmental in business needs within budget and targeted deadlines. Able to function in a complex environment. Must be flexible. Must have strong computer skills including use of Word, Excel, PowerPoint, Access, and Outlook.
