Physician Leader

HarmonyCares is seeking a Physician Leader in our Healthcare & Medical Services department. As a physician-led, home-based primary and palliative care organization, we serve medically complex, home-limited patients through interdisciplinary teams. In this role, you’ll provide clinical leadership, support high-quality in-home visits and telehealth, and guide care management to improve outcomes and reduce avoidable hospitalizations. You’ll foster a compassionate, patient-first, team-based culture while mentoring clinicians, collaborating across disciplines, and driving quality, access, and innovation in a mission-driven environment.
