As a Home Health RN Case Manager, you provide in-home skilled nursing care, manage individualized care plans, and coordinate services to help adults safely recover or manage chronic conditions at home. You conduct admissions and follow-up visits, collaborate with physicians and an interdisciplinary team, supervise home health aides, educate patients and caregivers, and maintain thorough clinical documentation to support quality, patient-centered care in the community.
