Licensed Medical Social Worker

Optimal Care is seeking a Licensed Medical Social Worker to provide compassionate, patient-centered support across our healthcare services. In this role, you will complete psychosocial assessments, deliver brief counseling and crisis intervention, coordinate discharge planning, and connect patients and families to vital community resources. You will collaborate closely with an interdisciplinary team of physicians, nurses, and therapists, advocating for patient needs and supporting advance care planning.
