CenterWell Home Health seeks a Home Health Speech Language Pathologist to deliver compassionate, evidence-based therapy in patients’ homes. You will perform comprehensive speech, language, cognitive, and swallowing evaluations; design and implement personalized treatment plans; and educate patients and caregivers to promote safety, autonomy, and quality of life. Collaborate closely with an interdisciplinary team, maintain accurate documentation, and uphold CenterWell’s values of compassion, integrity, and clinical excellence while contributing to continuous quality improvement.
