In-Home Nurse Practitioner

Signify Health seeks a Nurse Practitioner to perform comprehensive in-home evaluations and primary care services for adult and senior patients. You will conduct detailed health assessments, review medications, manage chronic conditions, and document findings in our EHR to support value-based care programs. Collaborating with interdisciplinary teams, you’ll identify gaps in care, make referrals, and educate patients and caregivers on treatment plans.
