Clinical Pharmacist II

Cone Health is seeking a Clinical Pharmacist II for our Acute Care/Hospital at Home program. In this role, you will provide comprehensive pharmaceutical care to acutely ill patients treated in the home or hospital setting, including order verification, medication therapy management, and patient counseling. You will collaborate closely with physicians, nurses, and care coordinators to optimize therapeutic outcomes, prevent adverse drug events, and ensure safe transitions of care.
