In-Home Primary Care Nurse Practitioner

As an In-Home Nurse Practitioner or Physician Assistant at Signify Health, you will perform comprehensive in-home evaluations for vulnerable patient populations, focusing on preventive, value-based care. You’ll review medical histories, perform physical exams, assess social determinants, and document findings in our advanced technology platform to close care gaps and support quality metrics. Collaborating with interdisciplinary teams, you’ll use data-driven insights to inform care plans, educate patients and caregivers, and help drive better outcomes while enjoying a mission-driven, flexible field-based role.
