Signify Health seeks an In-Home Traveler Nurse Practitioner or Physician Assistant to perform comprehensive clinical evaluations in patients’ homes. You will conduct detailed assessments, reconcile medications, identify care gaps, and document in EHR systems to support value-based care programs. Working with interdisciplinary teams and using modern technology, you’ll help vulnerable, often chronically ill patients improve outcomes and reduce hospitalizations.
