Requirements & Qualifications
- Active Connecticut RN license
- Graduate of an accredited nursing program (BSN preferred)
- Minimum 1 year of hospice experience
- Valid driver's license and reliable transportation
- CPR Certification
- Strong assessment, communication, and case management skills
- Computer proficiency with electronic medical records
Must Haves
- Manage a hospice caseload from admission through end-of-life care
- Perform comprehensive hospice assessments and develop individualized care plans
- Coordinate care with physicians and the Hospice Interdisciplinary Group (IDG)
- Provide symptom management, pain control, and end-of-life nursing interventions
- Educate patients and families on disease progression, medications, and the dying process
- Coordinate services with facilities, caregivers, and community resources
- Complete timely hospice documentation and regulatory compliance requirements
- Participate in weekend/holiday on-call rotation as scheduled
Preferred Background & Skills
- Previous hospice or palliative care experience
- Home Health or Visiting Nurse experience
- Strong end-of-life symptom management skills
- Medicare Hospice Conditions of Participation knowledge
- Case management experience
- Experience working with interdisciplinary care teams
- Excellent patient and family education skills
