Responsibilities:

This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families. Provides Social Work Practice coaching to assigned staff to promote the development of responsibility, skill, knowledge, and ethical standards in the practice of Social Work.

Qualifications:

Education

  • Master's Degree in Social Work Required

Work Experience

  • 2 years of experience in an acute hospital setting Required

Licenses and Certifications

  • LMSW - Licensed Masters Social Worker Upon Hire Required or
  • LCSW- License Clinical Social Worker Upon Hire Required
  • Accredited Case Manager (ACM) Upon Hire Required or
  • CCM - Certified Case Manager
