Position Overview
Birchwood Therapeutic Solutions is in search of a dedicated and compassionate Physical Therapist (PT) to join our team in Trumbull, Connecticut. This full-time position focuses on providing exceptional physical therapy services in a home care setting. Work Environment
The role is predominantly onsite, providing direct patient care in various home environments across Trumbull and surrounding areas. Our home care approach allows therapists to offer personalized services, directly in patients' living spaces, enhancing comfort and engagement in their recuperation process. The schedule for this position is Monday to Friday, allowing for a healthy work-life balance.
Key Responsibilities
- Conduct comprehensive evaluations of patients, including reviewing medical histories and performing specific functional assessments to identify individual needs.
- Develop, explain, and discuss detailed treatment plans with patients and their families, encouraging active participation in goal-setting for optimal recovery.
- Implement individualized treatment protocols grounded in clinical evidence and patient-centered care, adapting strategies based on each patient's unique progress and health status over time.
- Maintain open communication with interdisciplinary healthcare teams to ensure holistic patient care, effectively documenting treatment outcomes and any changes in patient conditions.
- Provide education to patients and families about therapeutic exercises and lifestyle modifications that can enhance recovery and overall health.
Patient Population / Client Focus
The Physical Therapist will work with a diverse patient population, including geriatric individuals recovering from surgeries, those with chronic illnesses, and patients with mobility impairments. Our focus on home care allows therapists to understand patient environments better, tailoring interventions that are not only effective but also contextually relevant to each individual's life.
Qualifications
- A Bachelor's degree in Physical Therapy from an accredited institution is required.
- State licensure or eligibility for licensure in Connecticut is a must.
- Strong interpersonal skills and the ability to communicate effectively with patients, families, and professional colleagues.
- Proficient in using electronic health record systems for documentation and progress tracking.
Preferred Experience
- Previous experience in home health care or outpatient settings is highly preferred.

Compensation and Benefits
The position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience.
