HarmonyCares seeks a compassionate Family Nurse Practitioner to deliver home-based primary and palliative care to medically complex, home-limited patients. In this role, you’ll conduct in-home and virtual visits, perform comprehensive assessments, manage chronic and acute conditions, and develop patient-centered care plans. Working within an interdisciplinary team, you’ll collaborate closely with physicians, nurses, social workers, and care coordinators to reduce avoidable hospitalizations and improve quality of life.
