Job Description:
Licensed Practical Nurse LPN Full Time Home Health

We are looking for a compassionate Licensed Practical Nurse to join our growing southern Maine team. This full-time position serves patients throughout York, Wells, and nearby communities.

What You’ll Do
Provide skilled nursing care in patients’ homes under the direction of an RN and physician
Follow individualized plans of care
Monitor patients and report changes in condition
Administer medications and treatments within your scope of practice
Educate patients and caregivers
Complete timely and accurate clinical documentation
Promote patient safety and independence
Collaborate with nurses, therapists, physicians, and other members of the care team
Participate in team meetings and the on-call rotation

What You’ll Need
Current and unrestricted Licensed Practical Nurse license in Maine
Graduate of an accredited practical nursing program
At least one year of experience in an acute care or comparable clinical setting
Current CPR certification through the American Heart Association or American Red Cross
Strong communication, organization, and documentation skills
Compassionate and patient-focused approach to care
Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Ability to travel between patient homes throughout Portland and Cumberland County
