RN Manager

IMMEDIATE need for an RN Manager who will coach in a collaborative environment that supports ongoing success. Area of focus will be MESSAGE MANAGEMENT, that includes communication and coordination, patient messages, and assisting with care plans. Will document patient interactions, relay information between patients and providers, use EHR to manage communications and follow-up care.

· Ensure staff answers patient questions, provide routine information, and act as a liaison between patients and the care team.

· Responsible for Message and Data Management, ensuring documentation of all patient encounters, advice, and outcomes accurately in the Electronic Health Record (EHR).

· Ensure staff support care plans, coordinate follow-up appointments, and refer patients to appropriate services.

· Responsible for triage and assessment, using triage protocols to assess patient needs and direct them to the appropriate level of care.

· Disseminate information, convey pertinent medical information to other departments to ensure continuity of care.

· Handle routine administrative requests and assist with population health initiatives as needed.

Required:

· Active CA RN license

· Preferred: Bachelor or Master degree in Nursing or Healthcare Administration

· Proficiency with EHR systems; EPIC preferred

· Strong organizational and multitasking abilities

· Problem-solving skills

· Attention to detail for accurate documentation

· Active CPR and BLS certifications
