In-Home Primary Care Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for health plan members in their homes. You will complete detailed assessments, review medical histories and medications, identify gaps in care, and document findings in an EHR to support value-based care. Collaborating with an interdisciplinary team, you’ll educate patients and caregivers, recommend follow-up care, and use data-driven tools to inform care plans. This role offers autonomy, flexible scheduling, and the opportunity to improve outcomes for vulnerable populations.
