Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for vulnerable patients in their homes. In this full-time field role, you’ll use data-driven insights to assess medical history, perform focused exams, close care gaps, and document in an EMR. You’ll collaborate with interdisciplinary teams to support value-based care, educate patients and caregivers, and improve outcomes.
