Healthcare Liaison

The Gables is seeking a compassionate Healthcare Liaison to support seniors and families across our assisted living and memory care communities in Utah and Idaho. In this resident-focused role, you will coordinate care transitions, support move-ins, and serve as the key communication bridge between families, residents, and our care teams. Responsibilities include assessments, education on services, documentation, and participation in family meetings and outreach. This position offers hands-on impact, collaborative teamwork, training, and opportunities to grow within senior living and community operations.
