As a Home Health Physical Therapist, you provide in‑home rehab services to adult patients in the far north/northeast suburbs. You assess mobility, safety, and equipment needs, create and adjust care plans, and deliver therapeutic interventions using home‑based equipment. You coordinate with nurses, physicians, and other team members, educate patients and caregivers, document visits in electronic systems, and travel between homes while managing your caseload and working some weekends/holidays.
