Full job description

As a Home Health Occupational Therapist, you provide in‑home evaluations and treatments to help adults safely improve independence in daily activities. You assess functional abilities and home safety, develop and adjust care plans, and select adaptive equipment to support mobility and self‑care. Working independently in patient homes, you document visits, educate patients and caregivers, and collaborate with nurses, physicians, and other therapists to coordinate quality, cost‑effective care across the continuum.
