RN Primary Case Manager

CarePartners, a Connecticut-based home healthcare provider, is seeking an RN Primary Case Manager to coordinate and deliver skilled nursing care in patients’ homes. In this role, you will manage a caseload of adult patients, complete comprehensive assessments, develop and update individualized care plans, provide direct clinical care, and educate patients and families on disease management and medications. You will collaborate closely with therapists, aides, and physicians, ensuring timely communication and documentation.
