RN Case Manager

CarePartners, a healthcare and medical services leader, is seeking a Registered Nurse Case Manager for our inpatient rehab setting. In this role, you will coordinate and manage patient care plans, collaborate with interdisciplinary teams, and ensure safe, effective transitions of care. You’ll advocate for patients, provide education to patients and families, and monitor progress toward rehabilitation goals. Ideal candidates have strong clinical assessment skills, excellent communication, and a passion for patient-centered, community-focused care in a collaborative environment.
