Heart Transplant Coordinator

Piedmont Healthcare seeks a Heart Transplant Coordinator to manage the full continuum of care for heart transplant patients within our Healthcare & Medical Services department. This role coordinates evaluation, listing, surgery, and long-term follow-up, serving as the primary liaison for patients and families. The coordinator partners closely with cardiology, surgery, and multidisciplinary teams to develop individualized care plans, ensure regulatory compliance, and support optimal outcomes.
