Post Lung Transplant Coordinator

Piedmont Healthcare is seeking a Post Lung Transplant Coordinator to manage care for lung transplant recipients. In this role, you will coordinate pre-discharge and post-transplant follow-up, monitor clinical status, manage complex medication regimens, and ensure adherence to treatment plans. You will provide patient and family education, facilitate multidisciplinary care conferences, and collaborate closely with surgeons, pulmonologists, pharmacists, and social workers. The coordinator will track outcomes, maintain accurate documentation, support quality improvement initiatives, and contribute to a culture of patient-centered, evidence-based care and continuous professional growth.
