Signify Health is seeking a full-time In-Home Traveler Physician to provide comprehensive, value-based clinical assessments for patients in their homes. You will travel regionally to perform detailed histories and physicals, review medications, identify care gaps, and document risk-adjusted conditions using modern technology. Partner with care teams to coordinate follow-up, improve outcomes, and reduce avoidable utilization.
