In-Home Traveler Nurse Practitioner or Physician Assistant

Signify Health is seeking In-Home Traveler Nurse Practitioners or Physician Assistants to perform comprehensive health evaluations for vulnerable patients in their homes. You will conduct head-to-toe assessments, review medical histories, reconcile medications, identify care gaps, and document findings in an EHR to support value-based care programs.
