In-Home Traveler Physician

Signify Health seeks a full-time In-Home Traveler Physician to deliver comprehensive in-home evaluations for vulnerable, often chronically ill members across multiple regions. In this role, you’ll travel to patient homes to perform head-to-toe assessments, reconcile medications, review social determinants of health, and document findings in an EHR to support value-based care and risk adjustment.
