Signify Health is seeking an In-Home Traveler Nurse Practitioner or Physician Assistant to perform comprehensive, value-based care evaluations for health plan members in their homes. You will conduct head-to-to-toe assessments, review medical histories, reconcile medications, and identify care gaps to support better outcomes and reduced costs. Working with modern tech tools and interdisciplinary teams, you’ll document in an EMR, communicate findings to care partners, and travel to varied communities while enjoying a collaborative, mission-driven, and flexible work culture focused on work–life balance.
