In-Home Nurse Practitioner

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive evaluations in patients’ homes. You will assess medical, functional, and social needs; review medications; identify care gaps; and develop evidence-based care plans that support value-based care. Using Signify Health’s technology platform, you’ll document findings, leverage data and analytics, and collaborate with interdisciplinary teams to coordinate follow-up care.
