Outpatient Nurse Practitioner or Physician Assistant

Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to provide patient-centered primary care in our multispecialty medical group in Latham, NY. In this role, you will evaluate and manage patients with acute and chronic conditions, order and interpret diagnostics, prescribe medications, and deliver education that supports long-term wellness. You will work closely with physicians, specialists, and care teams in a collaborative, physician-owned environment that values quality, work–life balance, and continuous improvement, with opportunities for growth across multiple outpatient centers.
