Full job description
A well-established long-term care organization is seeking an experienced MDS Coordinator (RN) to join our clinical leadership team. The ideal candidate is detail-oriented, organized, and knowledgeable in MDS processes and regulatory requirements. This position plays a key role in ensuring accurate clinical documentation, reimbursement, and quality resident care.
Key Responsibilities
Complete accurate MDS assessments and comprehensive care plans in accordance with regulatory requirements.
Initiate and update resident care plans while collaborating with the interdisciplinary care team.
Prepare and distribute monthly care plan schedules in a timely manner.
Maintain and update resident assessments and care plans as required.
Monitor and audit clinical documentation to ensure accuracy, completeness, and compliance.
Identify documentation concerns and communicate ongoing compliance issues to nursing leadership.
Maintain strict confidentiality of resident, employee, and organizational information.
Support quality assurance initiatives and contribute to positive survey outcomes.
Qualifications
Prior hands-on experience with MDS 3.0 is required.
Current Registered Nurse (RN) license in the applicable state.
Previous long-term care or skilled nursing experience is required.
Strong organizational, time management, and critical thinking skills.
Proficient with electronic medical records and computer applications.
Excellent verbal and written communication skills.
Ability to work collaboratively as part of an interdisciplinary team while managing multiple priorities.
