Centers Health Care is seeking an MDS Assessor to join our Information Technology & Software team. In this role, you will analyze, validate, and document Minimum Data Set (MDS) assessment data using specialized software tools to ensure regulatory compliance and data accuracy. You will collaborate with clinical and IT teams to translate assessment requirements into system configurations, generate reports, troubleshoot data issues, and support end users. You’ll help optimize workflows, contribute to process improvements, and participate in training to support a culture of learning, innovation, and collaboration.
