NURSE PRACTITIONER

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for patients in their homes. You will conduct physical exams, review medical histories, manage chronic conditions, reconcile medications, and document findings in our digital platforms. Collaborating with interdisciplinary teams, you’ll support care coordination and value-based care plans that improve outcomes for vulnerable populations.
