In-Home Health Assessment Nurse Practitioner

Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adults in their homes. In this role, you’ll conduct detailed histories and physicals, reconcile medications, assess social determinants of health, and identify gaps in care for vulnerable and chronically ill patients.
