In-Home Health Assessment Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to complete comprehensive health evaluations for members in their homes. You will perform head-to-toe assessments, review medical histories, reconcile medications, identify care gaps, and document findings in our EMR. Working with data-driven tools and a supportive clinical team, you’ll help drive value-based care, support chronic disease management, and connect patients with appropriate follow-up. This flexible role supports work–life balance while improving outcomes for vulnerable populations.
