CenterWell Home Health seeks a compassionate Home Health Licensed Practical Nurse to deliver skilled, in-home nursing care. The LPN will perform assessments, administer medications and treatments, manage wounds, and monitor chronic conditions while following individualized care plans. You will educate patients and families, promote safety and independence, and document care accurately in EMR systems. Working closely with an interdisciplinary team, you’ll support high-quality outcomes and patient satisfaction.
