Physician Assistant

Signify Health is seeking a Physician Assistant for in-home visits to deliver high-quality, value-based care to vulnerable patient populations. In this role, you will travel to patients’ homes to perform comprehensive health assessments, review medical histories, reconcile medications, and identify care gaps. You will use modern technology and EHR tools to document findings, develop personalized care plans, and coordinate with physicians, care coordinators, and health plans. Working within a collaborative, mission-driven culture, you’ll help improve outcomes, support preventive care, and contribute to continuous improvement in a flexible, patient-centered environment.
