Hospice Physician Designee

Heartland Hospice, part of ProMedica, is seeking a Hospice Physician Designee to provide medical oversight for hospice patients in home and facility settings. In collaboration with the hospice medical director and interdisciplinary team, you will conduct face-to-face visits, certify and recertify hospice eligibility, manage complex pain and symptoms, and develop individualized plans of care. You will communicate closely with families, primary physicians, and care staff, ensuring patient-centered, culturally sensitive end-of-life care aligned with comfort, dignity, and mission-driven values.
