In-Home Primary Care Nurse Practitioner

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adults and seniors in their homes. In this field-based role, you’ll assess chronic conditions, medications, and social determinants of health, then develop evidence-based care plans that close gaps in care and support value-based models. You’ll document visits using Signify’s technology platforms, coordinate with health plans and providers, and educate patients and caregivers.
