In-Home Health Assessment Nurse Practitioner or Physician Assistant

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for primarily Medicare and other vulnerable populations in their homes. You will complete detailed histories and physicals, review medications, identify care gaps, and document findings in an EHR to support value-based care and risk adjustment. Working part time, you’ll collaborate virtually with an interdisciplinary team, use modern mobile tools and analytics, and help connect patients with needed services while enjoying schedule flexibility and a mission-driven, supportive culture.
