Case Manager, Social Work

Kaiser Permanente is seeking a Case Manager, Social Work – In Patient II to support hospitalized members through comprehensive, patient-centered care coordination. In this role, you’ll conduct psychosocial assessments, develop and monitor care and discharge plans, and collaborate closely with physicians, nurses, and allied health staff to ensure safe transitions of care. You’ll advocate for patients and families, connect them with community and behavioral health resources, and help resolve barriers to recovery.
