LCSW Case Manager

Kaiser Permanente is seeking an LCSW Case Manager to deliver compassionate, patient-centered care within our Community & Social Services department. In this role, you’ll assess psychosocial needs, develop treatment and discharge plans, coordinate services, and advocate for patients and families across healthcare and community settings. You’ll collaborate closely with physicians, nurses, and external agencies to ensure continuity of care, reduce barriers to treatment, and support chronic disease and behavioral health management.
